| NOTICE: Effective April 1st Fannie Mae and FHA have released a new required form, the 1004MC, for all first mortgage originations (purchase & refinance) being SOLD TO Fannie, Freddie and FHA.
The new products with the 1004MC addendum are available on the Fiserv web site. To identify these products for ordering, the codes will contain the words “FNMA 1004 MC" in the product description. All FHA orders will contain the 1004MC addendum where the effective date of the appraisal is on or after April 1, 2009.
